• Tutor Employee Application If you would like to apply to be a tutor for the TRIO Program, please fill out this form and return it to the TRIO office in Freudenberger House, West Lower Level. View the list of criteria to be a tutor for our tutoring program and current salary.
• Tutor Job Description This form describes the Tutoring position, the applicant's required qualifications and lists specific duties and responsibilities.
• Class Assignments Tracking Sheet This form is used to track each assignment in a student's class and the score they received on that assignment.
• Grade Tracking Sheet This form is used to track an individual’s assignments and their overall grade in the course. This is the form that tutors are required to help their students fill out. If you are unsure about how to complete this form, please see the tutoring supervisor.
• Application To Request Tutoring Services If you are a currently active TRIO student and would like to request a tutor for one of your classes, please complete this form and return it Freudenberger House, West Lower Level.
• Weekly Tutor Report This is the Weekly Tutor Report Form that is required to be filled out by the tutor for each student actively involved in tutoring. Deadline is Friday each week by 4:00pm.
